Souhlasím s tím, že v den zkoušky předložím platný občanský průkaz nebo cestovní pas a souhlasím s tím, aby mě zkušební centrum v den písemné a/nebo ústní zkoušky vyfotografovalo. Také zmocňuji Cambridge English Assessment k bezpečnému uložení této fotografie a jejímu zpřístupnění pouze těm institucím nebo osobám, kterým jsem udělil/a souhlas. Prohlašuji, že jsem se seznámil/a s podmínkami registrace a budu se řídit pravidly stanovenými pro zvolenou zkoušku. Dále potvrzuji, že jsem seznámen/a s průběhem zkoušky od registrace až po obdržení certifikátu a se souvisejícími termíny.

Potvrzuji, že jsem zákonným zástupcem kandidáta uvedeného v tomto dokumentu a zplnomocňuji ho k absolvování dané zkoušky. Potvrzuji, že jsem si přečetl/a předpisy, které zahrnují předložení platného a aktuálního občanského průkazu nebo cestovního pasu v den zkoušky (pro kandidáty starší 14 let), a že centrum HDE může kandidáta v den zkoušky vyfotografovat. Dále beru na vědomí, že Cambridge English tuto fotografii bezpečně uloží a že bude k dispozici pouze těm institucím a/nebo osobám, které výslovně zmocním k přístupu k výsledkům zkoušky. Souhlasím s tím, že tyto instituce a/nebo osoby mohou tyto údaje použít k ověření výsledku zkoušky kandidáta. Prohlašuji, že jsem se seznámil/a s podmínkami registrace a budu se řídit pravidly stanovenými pro zvolenou zkoušku. Dále potvrzuji, že jsem seznámen/a s průběhem zkoušky od registrace až po obdržení certifikátu a se souvisejícími termíny.
